To Make a Report

You may use either of the following two methods to submit a report:

  • Select the "Make a Report" link at the top of this web page.

  • Dial toll-free, within the United States, Guam, Puerto Rico and Canada: 855-481-6238

After you complete your report you will be assigned a unique code called a "report key." Write down your report key and password and keep them in a safe place. After 5-6 business days, use your report key and password to check your report for feedback or questions.

EthicsPoint is NOT a 911 or Emergency Service:
Do not use this site to report events presenting an immediate threat to life or property. Reports submitted through this service may not receive an immediate response. If you require emergency assistance, please contact your local authorities.

Our Commitment

Xavier University is committed to the highest ethical standards. To help maintain those standards, the University has established simple, anonymous ways for you to confidentially report potential ethical misconduct or breaches of laws, rules, regulations, or Xavier University policies. You may report such matters using this web-based hotline or by the telephone hotline.

This hotline reporting service is intended to offer an alternative means of reporting for individuals who are unable to use existing reporting methods or who have found existing methods unsatisfactory. The hotline does not replace or supersede existing reporting methods on campus. The University strongly encourages employees and other members of the Xavier community to report relevant concerns directly to their supervisor, or through appropriate University offices as a first step whenever possible.

Xavier University assures that hotline reports will be handled promptly and discreetly. No retaliatory action will be taken against anyone for reporting or inquiring in good faith about potential ethical misconduct or breaches of laws, rules, regulations or Xavier University policies, or for seeking guidance on how to handle suspected misconduct or breaches.

The hotline is administered using a third party web site and telephone line staffed by EthicsPoint, a leading third party provider of confidential hotline reporting. Reports will be forwarded to three individuals at Xavier for follow-up: the Assistant Vice President for Audit and Risk Management, the Audit Manager, and the relevant administrator based on the category under which you file the complaint. You can report any concerns related to the administrator in the “Other” category to avoid it being automatically directed to the relevant administrator for the area.

For student conduct matters, when possible, please first report in accordance with the guidelines established in the Xavier Student Handbook prior to using this hotline as the first course of action.
