Make a Report Follow Up on a
Report

This system may be used by anyone who would like to report a concern to BCM. This includes faculty, staff, students, patients, research subjects and contractors/vendors.

After you complete your report, you will be assigned a unique code called a "report key." Write down your report key and password and keep them in a safe place as it cannot be reset. After 5-6 business days, use your report key and password to check your report for feedback or questions.

We strictly adhere to our BCM Code of Conduct, which contains guidelines for conducting business, research, education and clinical operations with the highest standards of professionalism and ethics.

The Integrity Hotline and Web Portal provides an additional method to report concerns. You may file a report by calling 855-764-7292 or selecting the “Make a Report” button at the top left. You may choose to report anonymously.

Baylor prohibits retaliation against anyone who reports concerns in good faith.
