After you complete your report, you will be assigned a unique code called a "report key." Write down your report key and password and keep them in a safe place. After 5-6 business days, use your report key and password to check your report for feedback or questions.

If you are a patient or family member, we encourage you to talk directly to the Center’s Director of Operations. If you are an associate, we encourage you to talk with your supervisor in circumstances where you believe there has been an incident of company policy or standards has occurred.

In situations where you prefer to place an anonymous report in confidence, you are encouraged to use this Helpline, hosted by EthicsPoint, a third-party helpline provider. You are encouraged to submit concerns regarding your care or perceived incidents of our Code of Conduct and Compliance Program Manual, as well as asking for guidance related to policies and procedures and providing positive suggestions and comments.

If you should choose, the information you provide will be sent to Urgent Care Group by EthicsPoint on a totally confidential and anonymous basis. You have our guarantee that your comments will be heard.
