Abuse of or Fraud with Benefits Improper, misleading or deceptive actions taken, falsification of records, or misrepresentation of physical conditions related to benefits plans including health and supplemental income plans, tuition reimbursement and sick or other paid time off programs.
Proper Accounting Concerns regarding questionable practices relating to accounting, auditing or internal financial controls. (Examples include, but are not limited to: misstatement of revenues or documents relating to revenue, misstatement of expenses, misstatement of assets, or misapplication of GAAP principles)
Confidentiality Confidentiality refers to the protection of the institution's and our customer's non-public information and use of such information only for legitimate business purposes.
Conflict of Interest A conflict of interest is defined as a situation in which a person, such as a public official, an employee, or a professional, has a private or personal interest sufficient to appear to influence the objective exercise of his or her official duties. (Examples include: inappropriate vendor relations, bribery, misuse of confidential information, inappropriate customer relations)
Discrimination Unlawful treatment on the basis of race, color, religion, sex, sexual orientation, sexual identity, age, disability, veteran's status, or national origin.
Falsification of Contracts, Reports or Records Falsification of records consists of altering, falsifying, or forging all or any part of a document, contract or record for the purpose of gaining an advantage, or misrepresenting the value of the document, contract or record. Failure to complete and post or file data and reports required by law to governmental agencies.
Fraud The intentional misrepresentation or concealment of information in order to deceive, mislead, or acquire something of value. A fraud is an intentional deception perpetrated to secure an unfair gain. (Examples include: Violation of Policy, Sabotage)
Harassment Uninvited or unwelcome physical or verbal conduct toward an individual that creates an intimidating, hostile, or offensive work environment, that unreasonably interferes with work performance, or that adversely affects employment opportunities.
Improper Disclosure of Financial Records Careless, unlawful or fraudulent conduct in recording, preparing, reporting, disclosing of either the value or the content of a contract, report, statement, document, record, or electronic file.
Improper Giving or Receiving of Gifts, Entertainment and Gratuities The giving, receiving or solicitation of items which could be reasonably interpreted as an effort to influence a business relationship or decision; items given, received or solicited for the benefit of an individual or an individual's family or friends; items given, received or solicited during or in connection with contract negotiations; the acceptance of cash, checks, money orders, vouchers, gift certificates, loans, products, or services.
Improper Supplier or Contractor Activity Supplier or contractor activity in violation of institution's policies and procedures; improper supplier or contractor selection based on personal gain, improper negotiation or diversion of contract awards.
Inappropriate Use of Technology Use of technology systems and/or equipment such as Internet, Electronic Mail, desktops, laptops, wireless devices, communications devices, radios, cellular/wireless telephones, and other computing devices in a manner that violates security, policy or guidelines.
Misconduct or Inappropriate Behavior Intentional wrongdoing; specifically: deliberate violation of a law or standard.
Misuse of Assets or Services Use of institutional resources or equipment without permission for non-institutional reasons.
Misuse of Resources Unauthorized use of corporately owned copyrights or software; also includes the use of unauthorized software on corporately owned systems.
Risk and Safety Matters Data privacy, environmental, health and safety concerns, unsafe working conditions, vandalism
Student Well-Being Bullying or cyber bullying, discrimination/harassment, hazing, retaliation, sexual misconduct, student safety, substance abuse
Theft/Embezzlement The act of stealing; specifically: the felonious taking and removing of personal property with intent to deprive the rightful owner of it.
Time Abuse Concerns about an employee or manager who is falsifying his/her work hours.
Violence or Threat Violence is an expression of the intention to inflict evil, injury, or damage to a person or their property. (Examples include: direct, veiled, conditional, violent)
Workers Compensation or Disability Benefits Abuses Observation of physical activity of employees receiving disability or other compensation benefits from the institution in contradiction to the degree of the disability certified by medical personnel.
