	Non Compliance with Established Timeframes	The reporter is aware that there is a task(s) within our policies and procedures that was not completed at its specific deadline as established in our policies and Procedures. For example, Claims paid out of the time as established in the Claims Department policies and procedure.
	Policy and/or Procedure not in Compliance with Federal and State Laws and Regulations	The reporter is aware that a specific Policy and Procedure is not in compliance with a Federal or State law and/or regulation. The regulation may be enacted or enforce by CMS, Medicaid agency or another regulator.
	Unauthorized Use of our applications and/or its Policies	The reporter is aware that one of our employees is not complying with the applicable policies and procedures. For example, an employee is printing policies and procedures without an express authorization.
