	Unauthorized Disclosure of PHI - External (MCS BAA)	An individual that disclose PHI without being authorized by the insured. To be authorized a Security and Privacy representative must sign the Authorization for use and disclosure form.
	Unauthorized Disclosure of PHI - Internal (MCS Employees)	An employee that disclose PHI to other internal employee without a business reason (TPO – treatment, payment or operation).
