Herzing University Student
File Complaint Select this form to file a complaint that is unrelated to grades or University policy. Typically a complaint would inform the University about issues related to the condition of the facilities, parking availability, textbooks or tuition-related concerns.
File Grievance Select this form to file a grievance resulting from University Failure to follow written policy or procedure, or an established practice. Students may initiate a grievance within 15 days from the event resulting in the complaint.
File Grievance Discrimination Select this form to file a grievance by which a student feels she/he has been discriminated against by Herzing University staff or faculty on the basis of sex, religion, color, creed, national origin, disability, age, or marital status. Students may initiate a grievance within 15 days from the event resulting in the complaint.
Report Violation of Code of Conduct Select this form to report a violation of the Student Code of Conduct including but not limited to academic misconduct, destruction of property, false report of emergency, manufacture, distribution, sale, offer for sale, possession or misuse of drugs or alcohol, mental or bodily harm to self or others, misuse or abuse of computers or computer networks, possession of weapons or dangerous materials ,stalking, theft or possession of stolen property or service, trespassing, use of tobacco products or electronic cigarettes in unapproved locations, and/or violation of criminal law. Sexual Harassment Select this from to report an instance of sexual harassment or sexual violence in any form that has occurred in the educational setting or off school grounds and/or outside of educational activities. Sexual harassment is inclusive of harassment based on gender identity, gender expression, and nonconformity with gender stereotypes as well as any unwelcome conduct of sexual nature. Unwelcome conduct of sexual nature includes unwelcome sexual advances or requests for sexual favors and other verbal, nonverbal or physical conduct of a sexual nature. Sexual violence includes, but is not limited to sexual assault, domestic violence/dating violence, and stalking. The harasser/perpetrator could be any member of the Herzing community including, but not limited to another student(s), Herzing University faculty or staff, or vendors or contractors. All such acts of sexual violence are forms of sexual harassment covered under title IX.
