Accounting, Auditing, Billing, and Internal Financial Controls Concerns about questionable practices related to accounting, auditing, billing or internal financial controls.
Conflict of Interest Any personal interest or relationship, prior or current employment, or any personal obligation that may interfere with the ability to objectively perform job duties and responsibilities or impair independence and objectivity or any financial interest, any business or professional activity, or any business or financial obligation that may interfere with the ability to objectively perform job duties and responsibilities or impair independence and objectivity.
Discrimination Statement or actions based on age, race, color, national origin, sexual orientation, gender, disability, or religion that are the basis for employment, promotion, or compensation decisions.
Falsification of Contracts, Reports or Records Falsification of records consists of altering, fabricating, falsifying, or forging all or any part of a document, contract or record for the purpose of gaining an advantage, or misrepresenting the value of the document, contract or record.
Gifts, Bribes and Kickbacks Payments, payments in kind, gifts, bribes, extensions of credit or benefits extended to or received by customers, employees, suppliers, vendors, competitors, directors, officers, auditors, government employees, government officials or agencies, or other parties that are unlawful, improper, or designed to influence business decisions or political processes.
Harassment Statements or actions expressing unwelcome sexual advances, requests for sexual favors, unsolicited physical contact or propositions, unwelcome flirtations, or offensive verbal or visual expressions, or physical conduct of a sexual nature. Persistent statements, conduct or actions that are uninvited, degrading, offensive, humiliating, or intimidating, and create an unpleasant or hostile work environment.
Hostile Work Environment An environment created by a supervisor or coworker whose actions, communications, or behavior alters the terms, conditions, and/or reasonable expectations of a suitable work environment for employees. The behavior or communication must be pervasive, lasting over time, and not limited to an occasional remark or two that an employee found annoying. In addition, to pervasiveness, the hostile behavior, actions or communication must be severe enough to disrupt the employee's work and harmful to the point that a reasonable worker could be dissuaded from engaging in protected activities.
Management Concerns related to the management or mismanagement of Laboratory/DOE programs, procedures, processes, employees, etc.
Misconduct / Inappropriate Behavior Intentional wrongdoing; specifically: deliberate violation of a law or standard.
Retaliation Any action taken against an employee in response to, or in revenge for, the employee having raised, in good faith, reasonable concerns about any aspect of DOE-related operations.
Safety, Health and the Environment Failure to provide a safe working environment or training required by company policies; failure to report accidents, etc. concealing of actual or potential environmental damage; failure to comply with environmental laws.
Scientific Integrity / Misconduct / Professional Opinion Lack of ethical responsibility for the proper care of research subjects and the quality and integrity of the scientific process.

Theft The act of stealing; specifically: the felonious taking and removing of personal property with intent to deprive the rightful owner of it.
Time Billing Concerns about an employee or manager who is falsifying his/her work hours.
Unauthorized/Fraudulent Use of Company Facilities and Equipment The misuse/abuse of company support services, equipment, or assets.
Violence or Threat Violence is an expression of the intention to inflict evil, injury, or damage to a person or their property. (Examples include: direct, veiled, conditional, violent).
Other Statements, actions or policies that are of concern but are not currently resulting in harm, injury or corporate liability and cannot be included in any other category.
