USF has engaged EthicsPoint—a third party hosted hotline—to enable the safe, secure, and anonymous reporting of activities which may involve misconduct, fraud, abuse, and other violations of USF policies. You may file a report by clicking on the To Make a Report link on the chart or by calling toll-free 1-866-974-8411.

USF is committed to providing a safe, secure, and ethical place to work. As a member of the USF community, you have a responsibility to voice compliance and ethics concerns, so that we can work together to promptly resolve the issues while helping to cultivate a positive environment.

The USF Policy 0-020: Retaliation, Retribution, or Reprisals prohibits retaliatory action be taken against anyone who reports or inquires in good faith about potential breaches of USF policies or seeks guidance on how to handle suspected breaches.

Reports made in EthicsPoint and associated investigatory records will be kept confidential to the extent possible under applicable state and federal law. Be advised that revealing your identity in EthicsPoint may result in disclosure of your identity in response to a public records request under Florida’s public records law [Chapter 119, Florida Statutes].

The following should NOT be reported in EthicsPoint:

• Academic Matters involving faculty and/or students should be reported directly to the Office of the Provost and Executive Vice President.
• Non-Academic Student Conduct Matters should be reported directly to Student Conduct & Ethical Development or by calling 813-974-9443.
