SCAN Health Plan is an organization dedicated to keeping Medicare beneficiaries healthy and independent at home. An integral part of our mission is the ability for our employees, members, providers, vendors and other affiliates to have the ability to report potential issues of non-compliance or fraud, waste or abuse in a manner that is either confidential or anonymous. We rely on each other to ensure organizational compliance to further our mission and vision.
We have selected EthicsPoint to provide a confidential and anonymous method to receive your suggestions, questions, concerns, and complaints of potential issues of non-compliance. This includes if you have a question or are unclear regarding an issue of potential non-compliance. Please Speak Up! and file a report so that we can investigate and respond to the information you provide.
The information you report will be sent to us by EthicsPoint on a confidential basis. You may also elect to submit this information on an anonymous basis when you fill out the report.
Remember: SCAN prohibits retaliation against anyone who makes a report in good faith.
To Report a Concern or Complaint
You may use either of the following two methods to submit a report:

  • Select the Make a Report link on the left side of this web page and select the option that best aligns with your concern or complaint.
To Make a Suggestion or Inquiry
You may use either of the following two methods to make a suggestion or inquiry:

  • Select the Make a Report link on the left side of this web page. Find and select the "Other" option for "Suggestion, concern or other inquiry"
What Happens after you submit a report?
After you complete your report you will be assigned a unique code called a "report key". Be sure to write down your report key and password and keep them in a safe and secure place. Your report will be received and assessed by the appropriate individual(s) at SCAN, and investigated further as necessary. After seven (7) business days, select the Follow-up on a Report link on the left side of the web page and use your report key and password to check the status of your report.
