To Make a Report

You may use either of the following two methods to submit a report:
  • Dial toll-free, within the United States, Guam, Puerto Rico and Canada:

  • Select the "Make a Report" link at the top of this web page.
This process is not intended to replace or supersede normal managerial and Human Resources communication channels.

After you complete your report you will be assigned a unique code called a "report key." Write down your report key and password and keep them in a safe place. After 5-6 business days, use your report key and password to check your report for feedback or questions.

If you do not feel comfortable doing this or are unsure of whom to report your concern, you can begin the reporting process using this web site or by calling the CSS Compliance Helpline at 1-888-664-9041. This reporting process is available 24 hours a day, 7 days a week.

Whistleblower Protection:

CSS does not tolerate any reprisals against people who make complaints. CSS protects whistleblowers who are board or staff members, participants and families, as well as other individuals. No one will be discharged, demoted, suspended, threatened, or discriminated against because of any lawful act done by employees, families, or persons served to provide information, cause information to be provided, or otherwise assist in an investigation regarding any conduct which the person making the report reasonably believes constitutes a violation of the ethical code of conduct.
