Request Guidance or Report a Concern
You may request guidance on UIC’s Code of Business Ethics and Conduct or report a claim or concern in one of the following three ways:
- Look to your supervisor or any member of UIC management
- Call 1-888-373-8855
- File an online claim
After you complete your claim you will be assigned a unique code called a "claim key." Write down your claim key and password and keep them in a safe place. Use your claim key and password to check your claim for feedback or questions.
