To Make a Report

You may use either of the following two methods to submit a report:

  • Dial toll-free, within the United States, Guam, Puerto Rico and Canada: 888-297-9331
  • OR
  • Select the "Make a Report" link at the top of this web page.
This process is not intended to replace or supersede normal managerial and Human Resources communication channels.

After you complete your report you will be assigned a unique code called a "report key." Write down your report key and password and keep them in a safe place. After 5-6 business days, use your report key and password to check your report for feedback or questions.

The Board and management value your input and have selected EthicsPoint to provide an anonymous and confidential method for us to hear your suggestions, concerns, or reports of misconduct. The information you provide will be sent to NYSID on a totally confidential and anonymous basis by EthicsPoint. You do not need to identify yourself and your communication will be kept confidential to the extent possible while allowing NYSID to investigate and respond.
