To help us maintain those standards, we have implemented an Ethics Line as a resource for our employees, clients, suppliers, and service providers to report events at any MidWestOne subsidiary. You may choose to include your contact information or submit your report anonymously.

Potential reportable events may include: inappropriate disclosure of confidential or personal, non-public information; conflicts of interest; seeking or accepting anything of value in exchange for loans or other bank services; harassment; theft; embezzlement or misappropriation of funds or assets; extortionate extensions of credit; unauthorized issuance of obligations or false entries; making false statements or loans to any Federal bank examiner; certifying a check drawn on an account in which there are not sufficient collected funds; lending to any officer, director, or employee of any firm engaged primarily in the issue, public sale or underwriting of stocks, corporate bonds, or other securities while an officer, director, or employee of a bank; or serving as an accessory after the fact of an offense.

Your trust and confidence is our highest priority. An investigation will be conducted and appropriate actions taken.

To Make a Report

You may use either of the following two methods to submit a report:

  • Dial toll-free, within the United States, Guam, Puerto Rico and Canada:

After you complete your report you will be assigned a unique code called a "report key." Write down your report key and password and keep them in a safe place. After 5-6 business days, use your report key and password to check your report for feedback or questions.
