A reporter's identity will be protected, except as necessary for the investigation or when disclosure is required by law. Anonymous reports will be accepted, but may inhibit investigations in some situations.

HOW TO FILE A REPORT
You may use either of the following two methods to submit a report:
Call 877-252-8856 (toll-free, within the US and Canada)
OR
Select the "File a Report" link at the top of this web page.
After you complete your report you will be assigned a unique code called a "report key." Write down your report key and password and keep them in a safe place. After 5-6 business days, use your report key and password to check your report for feedback or questions.
