To Make a Report

You may use either of the following two methods to submit a report:

After you complete your report, you will be assigned a unique code called a "report key". Write down your report key and password and keep them in a safe place. After 5-6 business days, use your report key and password to check your report for feedback or questions.

This website allows you to make reports using your name or anonymously if you choose. EthicsPoint, the company that operates this site and the Compliance Help-Line, will forward your report to Northwell on a confidential basis. You can make a report on this website or you can call the Compliance Help-Line at 800-894-3226 24/7. You are protected against retaliation for reporting concerns or asking questions related to suspected or actual compliance violations.
