To Make a Report
You may use either of the following two methods to submit a report:

After you complete your report you will be assigned a unique code called a "report key." Write down your report key and password and keep them in a safe place. After 5-6 business days, use your report key and password to check your report for feedback or questions.
The goal of MMM is to behave ethically and with integrity as a leader in the healthcare industry. That is why we encourage you to take action upon the witness or suspect of any noncompliant or unethical conduct.
The main objective of MMM is to comply with reporting requirements, identify risk areas, prevent misconduct and operational inefficiencies, enhance operational functions, improve the quality of health care service, and decrease the cost of health care.
Examples of non-compliant or unethical behavior:
- A violation of any company policy.
- A violation of any law, rule or regulation.
- A violation of security, privacy or criminal conduct.
- Fraud, misconduct, questionable practices, internal business.
- Bribery or falsification of any medical record that is property of the company.
- Retaliation or abusive conduct, threats of violence within the workplace.
- Violations from upper management or colleagues.
- Conflicts of interest or unfair treatment.
If you are aware, or become aware of any unethical or non-compliant behavior, please let us know by clicking “Make a Report”. There you will be able to submit a report through the web-based reporting system, EthicsPoint. The EthicsPoint web-based reporting system is a method available for providers, contractors, beneficiaries, among others, to report any illegal or unethical conduct, or otherwise inconsistent with our Compliance Program.
It is your choice to submit a report on a confidential or anonymous basis. However, note that it is often helpful to provide contact information in order to facilitate the investigation process.
