To Make a Report

You may use either of the following two methods to submit a report:

  • Select the "Make a Report" link at the top of this web page.

  • Dial toll-free, within the United States, Guam, Puerto Rico and Canada:

After you complete your report you will be assigned a unique code called a "report key." Write down your report key and password and keep them in a safe place. After 5-6 business days, use your report key and password to check your report for feedback or questions.

EthicsPoint is NOT a 911 or Emergency Service:

Do not use this site to report events presenting an immediate threat to life or property. Reports submitted through this service may not receive an immediate response. If you require emergency assistance, please contact your local authorities.

Van Andel Institute (VAI) is committed to fostering and maintaining the highest standards of integrity and ethical conduct in all activities. As part of the commitment to maintaining high integrity and ethical conduct within the Institute, VAI has contracted with Ethics Point, an external reporting service, for a means to anonymously and confidentially report issues and concerns to management.

All reports submitted through EthicsPoint will be handled as promptly and as discreetly as possible. The report will be made available only to those at VAI who need to know to assess and resolve the matter. Concerns or issues that can be reported through EthicsPoint include but are not limited to: financial, personnel, research misconduct, human or animal subjects research, intellectual property, purchasing, safety, security, sponsored research, transfer or exports of materials and data.
