To Make a Report

You may use either of the following two methods to submit a report:

  • Select the "Make a Report" link at the top of this web page.
  • OR
  • Dial toll-free, within the United States, Guam, Puerto Rico and Canada:

After you complete your report you will be assigned a unique code called a "report key." Write down your report key and password and keep them in a safe place. After 5-6 business days, use your report key and password to check your report for feedback or questions.

FSU Hotline

The Florida State University has selected EthicsPoint to provide you with a simple way to confidentially report activities that involve potential misconduct or violations of FSU policies and procedures, regulations, or state and federal laws.

This hotline service allows you to report incidents of the following nature:

  • Fraud, Abuse, Theft, or Embezzlement
  • Economic Waste, or Misuse of University Resources
  • Unethical Behavior and Dishonest Acts
  • Conflicts of Interest
  • Public or Environmental Health and Safety
  • Discrimination or Harassment, Sexual Harassment
  • Non-compliance with University Policies related to the above

Reports submitted to EthicsPoint will be handled promptly and discreetly. No retaliatory action will be taken against anyone for making a report or inquiry to EthicsPoint in good faith.

EthicsPoint has been established in addition to existing reporting systems on campus and is not intended to replace management or supervisory oversight. You may report your concerns directly to your supervisor or manager as appropriate to resolve problems.

Potential Whistle-blower complaints: If you feel that your complaint meets the criteria for Whistle-blower in accordance with 112.3187-112.31895, Florida Statute or to make a report directly to the FSU Office of Audit and Advisory Services (OAAS) click the OAAS link in the "Quick Links" menu. The OAAS is responsible for reviewing all potential Whistle-blower allegations for the University. An individual cannot self-designate as a Whistle-blower, as that determination must be made by the OAAS. Additionally, a potential Whistle-blower cannot be anonymous as certain statutory criteria must be met.
