To Make a Report

You may use either of the following two methods to submit a report:

  • Select the "Make a Report" link at the top of this web page.
  • OR
  • Dial toll-free, within the United States, Guam, Puerto Rico and Canada:

After you complete your report you will be assigned a unique code called a "report key." Write down your report key and password and keep them in a safe place. After 5-6 business days, use your report key and password to check your report for feedback or questions.

FSU Hotline

The Florida State University has selected EthicsPoint to provide you with a simple, anonymous way to confidentially report activities that involve certain improper conduct or violations of FSU policies and procedures, regulations, or state and federal laws.

This hotline service allows you to report incidents of the following nature:

  • Fraud, Theft or Embezzlement
  • Economic Waste, or Misuse of University Resources
  • Conflicts of Interest
  • Computer Security and other Privacy Violations
  • Public or Environmental Health and Safety
  • Discrimination or Harassment, Sexual Harassment
  • Compliance with University Policies

Reports submitted to the hotline will be handled promptly and discretely. No retaliatory action will be taken against anyone for reporting or inquiring in good faith about potential breaches of FSU policies or seeking guidance on how to handle suspected breaches.

The FSU Hotline has been established in addition to existing reporting systems on campus. This service is not intended to replace management or supervisory oversight. You may always report your concerns directly to your manager. Individuals are also encouraged to bring concerns to their supervisor, the Office of Inspector General Services, the Office of Equal Opportunity and Compliance, or other campus entities as appropriate to resolve problems or disputes through established channels.

Please note: This is not a 911 service. If you need to report a possible criminal activity, or you are concerned about an immediate threat to life or property please call law enforcement at 911 or 850-644-1234.
