To Make a Report

You may use either of the following two methods to submit a report:

  • Dial toll-free, within the United States, Guam, Puerto Rico and Canada:

After you complete your report you will be assigned a unique code called a "report key." Write down your report key and password and keep them in a safe place. After 5-6 business days, use your report key and password to check your report for feedback or questions.

The NIC Reporting Hotline is an anonymous reporting mechanism that facilitates reporting of possible illegal, unethical or improper conduct, whether involving the company’s employees, its contractors, its suppliers, or the Company’s officers or directors. Often the Reporting Hotline is used when the normal channels of communication have proved ineffective, or are impractical under the circumstances. The Audit Committee of the NIC Board of Directors oversees the process for handling Reporting Hotline complaints.

The Audit Committee is made up of independent directors who are not affiliates or officers of the Company. In order to facilitate reporting, the Audit Committee has established procedures for (1) the receipt, retention and handling of reports and (2) the confidential, anonymous submission by employees or others of reports or concerns regarding the items listed above. If a Reporting Hotline report is made, it will remain anonymous unless the individual making the report voluntarily provides his or her name. With or without a name, reports are confidential to the extent reasonably possible.
