To Make a Report

You may use either of the following methods to submit a report:

  • Select the "Make a Report" link at the top of this web page and submit electronically.
  • OR
  • Dial toll-free, within the United States, Guam, Puerto Rico and Canada: 855-363-0857

After you complete your report you will be assigned a unique code called a "report key". Write down your report key and password and keep them in a safe place. We encourage you to use the report key and password and return to this website often (especially within the first few days of making a report). The University may have information for you or may need additional information from you to look into your concern. Communications through this process are confidential and allows for continued anonymity. Following up on a report will also allow you to see whether your reported matter is resolved. Due to confidentiality requirements, you may not necessarily be informed as to how the matter was resolved.

EthicsPoint is NOT a 911 or Emergency Service
Do not use this site to report events presenting an immediate threat to life or property. Reports submitted through this service may not receive an immediate response. If this is an emergency and/or there is a potential threat to you or someone else, please dial 911 or call Mines Public Safety (campus police) at 303-273-3333.
