To Make a Report

You may use either of the following two methods to submit a report:

  • Dial toll-free, within the United States, Guam, Puerto Rico and Canada: 855-384-9880

After you complete your report you will be assigned a unique code called a "report key." Write down your report key and password and keep them in a safe place. After 5-6 business days, use your report key and password to check your report for feedback or questions.

EthicsPoint is NOT a 911 or Emergency Service:

Do not use this site to report events presenting an immediate threat to life or property. Reports submitted through this service may not receive an immediate response. If you require emergency assistance, please contact your local authorities.

This reporting tool gives member-owners, employees, customers, applicants, business partners and vendors the opportunity to report, confidentially and anonymously, any complaints relating to potential accounting irregularities, financial misappropriations, or other violations of law, regulation, or policy. Reporting matters could include allegations of corruption, fraud, operational weaknesses, or unethical behavior. The whistleblower program is not intended for making comments or complaints regarding customer service matters.

Those who are aware of potential fraud, unlawful, or unethical behavior relating to FCS Financial are encouraged to report their concerns. It is the responsibility of all board members, officers and employees to report activities that may violate laws, regulations, policies, or ethical guidelines.

Reports can be submitted by telephone or by clicking the link provided. The report you submit to EthicsPoint will be provided to the FCS Financial Board of Director Chair person and Audit Committee Chair and Vice-Chair in a completely confidential and anonymous manner. Upon their review and investigation, they will provide a confidential response to your report that you will be able to follow-up on. You have our assurance that your report will be anonymous and your comments will be heard and responded to.

This system is not a substitute for routine communications regarding complaints or suggestions for improvement that do not involve a potential violation of laws, regulations, policies or ethical guidelines. Communications regarding regular business matters that do not require anonymity should be directed to the appropriate board member or employee of FCS Financial and should not be made through this service.
