To Make a Report

You may use either of the following two methods to submit a report:

  • Select the "Make a Report" link at the top of this web page.
  • OR
  • Dial toll-free, within the United States, Guam, Puerto Rico and Canada: 855-653-7071

After you complete your report you will be assigned a unique code called a "report key." Write down your report key and password and keep them in a safe place. After 5-6 business days, use your report key and password to check your report for feedback or questions.

EthicsPoint is NOT a 911 or Emergency Service:
Do not use this site to report events presenting an immediate threat to life or property. Reports submitted through this service may not receive an immediate response. If you require emergency assistance, please contact your local authorities.

Nazareth University is an institution of higher education with strong values of responsibility and integrity. We strive to conduct all College work matters in line with our core values. Nazareth University employees, including student workers, are expected to abide by state and federal laws and regulations as well as College policies in carrying out their duties and responsibilities for the College. All College employees, including student workers, cannot be compelled by a supervisor or College official to violate the law or College policies. An individual who has knowledge of specific acts(s) which he or she reasonably believes violate(s) the law or College policy has a responsibility to disclose that information. The Nazareth University Reporting of Wrongful Conduct (Whistleblower Policy) contains general guidelines for conducting College work with the highest standards of ethics.

Nazareth University is committed to an environment where open, honest communications are the expectation, not the exception. We want you to feel comfortable in approaching your supervisor or management in instances where you believe violations of policies or standards have occurred.

In situations where you prefer to place an anonymous report in confidence, you are encouraged to use the Nazareth Ethics Hotline, hosted by a third party hotline provider, EthicsPoint. This system will complement existing campus reporting options for financial, accounting and other compliance related violations. You are encouraged to submit reports relating to violations stated in our Nazareth University Reporting of Wrongful Conduct (Whistleblower Policy), as well as asking for guidance related to policies and procedure. You may submit a report through this site or by calling (855-653-7071). The hotline can be used for either direct or anonymous reporting. There are other methods of reporting as outlined in the Nazareth University Reporting of Wrongful Conduct (Whistleblower Policy).

This hotline should not be used to report health, fire and safety, personnel or academic matters.

Reports not accepted via the hotline include the following:

  • Human Resources issues such as performance, sexual harassment or discrimination. Complaints concerning prohibited harassment or discrimination should be reported using the procedures set out in the Non-Discrimination Policies and Procedures for faculty, staff and students or contacting Human Resources at 585-389-2065.
  • Issues concerning sexual or physical violence. Please contact one of these offices:

  • Emergency or Public Safety Concerns. Please contact Campus Safety: On-Campus, x3333, off campus, 585-389-2850. If you require emergency assistance, please call 911.

The information you provide will be sent to us by EthicsPoint on a totally confidential and anonymous basis, if you should choose. The College will investigate all reports submitted promptly and discreetly. You have our guarantee that your comments will be heard. The College will not tolerate retaliation toward, or harassment of, anyone who makes a good faith effort to appropriately disclose an alleged impropriety through this method.
