You may use either of the following two methods to submit a report:
After you complete your report you will be assigned a unique code called a "report key." Write down your report key and password and keep them in a safe place. After 5-6 business days, use your report key and password to check your report for feedback or questions.
Do not use this site to report events presenting an immediate threat to life or property. Reports submitted through this service may not receive an immediate response. If you require emergency assistance, please contact your local authorities.
Mission Community Hospital places great emphasis on the need for responsibility and integrity in the workplace. Our Code of Conduct contains general guidelines for conducting business with the highest standards of ethics.
At Mission Community Hospital, we are committed to an environment where open, honest communications are the expectation, not the exception. Should the need arise, we want our employees to feel comfortable in approaching supervisors or management in instances where they believe violations of policies or standards have occurred.
Should an employee prefer to place an anonymous report in confidence, we have created a third party hotline through EthicsPoint. Employees are encouraged to submit reports relating to violations stated in Code of Conduct, ask for guidance relating to policies and procedure, and provide positive suggestions and stories relating to issues of ethical conduct in the workplace.
Any information provided will be sent to us by EthicsPoint on a confidential and anonymous basis. You have our guarantee that your good faith comments will be heard and pertinent issues will be addressed without repercussion.
See the EthicsPoint FAQs for more information.
