Make a Report

If you have a bioethics issue or need to schedule an ethics consult, please call (912) 350-8681, otherwise you may use either of the following two methods to submit a report:

  • Select the link above.
  • OR
  • Dial toll-free, within the United States, Guam, Puerto Rico and Canada: 800-555-8497

After you complete your report you will be assigned a unique code called a "report key." Write down your report key and password and keep them in a safe place. After 5-6 business days, use your report key and password to check your report for feedback or questions.

NAVEX is NOT a 911 or Emergency Service:
Do not use this site to report events presenting an immediate threat to life or property. Reports submitted through this service may not receive an immediate response. If you require emergency assistance, please contact your local authorities.

Memorial Health is committed to maintaining an environment where open, honest communications are the expectation, not the exception. We want you to feel comfortable raising questions and concerns including instances where you believe violations of standards or policies may have occurred.

In some situations you may prefer to use this hotline, hosted by a third party hotline provider, NAVEX Global (“NAVEX”). You may submit reports relating to violations of our written standards and policies, as well as asking for guidance related to compliance, business conduct or integrity.

The information you provide to NAVEX will be sent to us in a format that protects your confidentiality. We take all reports about compliance and integrity seriously and will look into each report and follow up when further action is appropriate.
