To Make a Report

You may use the following two methods to submit a report:

After making the report, an EthicsPoint call center attendant will assign you a unique code called a "report key" and ask you to create a password. Write them down and keep them in a safe place. You will use the report key and password to check your report for CleanSlate feedback or questions.

The Reportline and EthicsPoint is NOT a 911 or Emergency Service. This site cannot be used to report events presenting an immediate threat to life or property. If you require emergency assistance, please contact your local authorities.

CleanSlate’s Reportline is not to be used to provide positive suggestions, stories or for general service complaints – please contact CleanSlate’s Call Center at 1-800-NEW-START for that purpose.

In situations where an employee or someone else prefers to place an anonymous report in confidence, we encourage them to use our CleanSlate’s Reportline hosted by EthicsPoint. This Reportline is used to submit reports to CleanSlate that allege violations of law or CleanSlate policy. The information provided in a report will be sent to CleanSlate by EthicsPoint on a totally confidential and anonymous basis, if you should choose. All reports are reviewed and acted upon – guaranteed.

Note – the Reportline should not be used for general service complaints or to share positive stories – please contact CleanSlate’s Call Center at 1-800-NEW-START for that purpose. Also, this hotline should not be used for medical emergencies – call 9-1-1.
