You may use either of the following two methods to submit a report:
After you complete your report you will be assigned a unique code called a "report key." Write down your report key and password and keep them in a safe place. After 5-6 business days, use your report key and password to check your report for feedback or questions.
As part of its commitment to ethics and compliance, Cleveland State University has contracted with EthicsPoint to provide a confidential reporting tool for CSU faculty, staff and students to anonymously report misconduct in the workplace or classroom setting due to mismanagement of funds, fraud, abuse or other violations of law or University policy.

CSU is a public institution subject to Ohio’s public records law. Protected disclosures and investigatory records will be kept confidential to the extent possible, consistent with the need to conduct an adequate investigation, and in accordance with the Ohio Public Records Act. If you choose to disclose your identity while submitting a report, we may not be able to protect your identity should a public records request be made.
