To Make a Report

After you complete your report you will be assigned a unique code called a "report key." Write down your report key and password and keep them in a safe place. After 5-6 business days, use your report key and password to check your report for feedback or questions.

Employees and clients of The Providence Services Corporation and its subsidiaries (including The Ingeus Group, LogistiCare and Matrix Medical Network; the “Company”), as well as any other persons, are encouraged to use this website to report any conduct or practice which appears to be or is unlawful, unethical or contrary to the business practices of the Company. Information may also be provided by telephone, by calling 855-818-6929. Both this website and the telephone hotline are hosted by a third party provider, EthicsPoint. Information may be provided anonymously if anonymous reporting is legally permitted in your country (if your country does not allow for anonymous reporting, you will be notified before submitting the report).
