After you complete your report you will be assigned a unique code called a "report key." Write down your report key and password and keep them in a safe place. After 5-6 business days, use your report key and password to check your report for feedback or questions.

In situations where you prefer to place an anonymous report in confidence (to the extent permissible by law) or otherwise prefer not to report the concern through your local supervisor or management, you are encouraged to use this hotline, hosted by a third-party hotline provider, EthicsPoint. You may also use this hotline to seek guidance relating to the Principles of Conduct and associated policies and procedures.
There will be no retaliation against anyone who reports a concern or provides information in good faith. Subject to duties arising under applicable law, all submissions will be treated as confidential. We have worked hard at Shape to develop a reputation of quality, excellence, integrity, and customer satisfaction. If you believe there has been a potential violation of the Shape 49015, we want to hear from you.
