To Make a Report

You may use either of the following two methods to submit a report:

  • Select the "Make a Report" link at the top of this web page.
  • OR
  • Dial toll-free, within the United States, Guam, Puerto Rico and Canada:

After you complete your report you will be assigned a unique code called a "report key." Write down your report key and password and keep them in a safe place. After 5-6 business days, use your report key and password to check your report for feedback or questions.

Our Commitment

Suffolk Care Collaborative is committed to demonstrating the ethical values of honesty, integrity, and accountability in all of our activities. Our Code of Conduct provides general guidance for everyone associated with us in conducting our business and activities according to these values, our compliance policies and procedures and all applicable governing frameworks.

We are committed to maintaining an environment where open, honest communications are the norm. To fulfill the duty to report known or suspected noncompliance, you may approach your supervisor, a manager, or your Compliance Officer. In situations where you prefer to place an anonymous report in confidence, however, you are encouraged to use this hotline. It provides both online and operator-assisted telephone reporting features and is hosted by a third-party provider, EthicsPoint/NAVEX Global.

The information you provide will be sent only to the Suffolk Care Collaborative Compliance Office by EthicsPoint in a totally confidential manner, and if you so choose, anonymously.

You may also use EthicsPoint to ask for guidance related to policies and procedure and providing positive suggestions and stories. You should not, however, use EthicsPoint in an emergency, as it is not a 911 line.
