You may use either of the following two methods to submit a report:
After you complete your report you will be assigned a unique code called a "report key." Write down your report key and password and keep them in a safe place. After 5-6 business days, use your report key and password to check your report for feedback or questions.

NC State is committed to an environment where open, honest communications are the expectation, not the exception. NC State wants all members of the university community, regardless of their position, to feel comfortable in approaching a supervisor or manager with their concerns.
NC State encourages the campus community to report concerns directly to a supervisor or manager through the offices responsible for investigating these concerns. These offices can include the Office of Institutional Equity and Diversity (OIED), Human Resources (HR), Internal Audit Division (IAD), or other responsible offices as appropriate. In addition, the EthicsPoint hotline is available. The EthicsPoint hotline supplements existing reporting mechanisms, providing an additional means of reporting. If you prefer to file a report using EthicsPoint, select the “Make a Report” link at the top if this webpage and follow the instructions.
Neither NC State nor EthicsPoint tracks the origin of online reports. If you choose, the information you report can be submitted anonymously. You are not required to enter your name or contact information when submitting an anonymous report through EthicsPoint. However, please provide as much detailed information about your concern as possible. Due to the nature of certain reports, NC State may be limited in its ability to follow-up on anonymous reports where it cannot ask additional questions of the reporting party.
NC State is committed to protecting individuals who report concerns. Reporting suspected violations of laws, policies, regulations or rules is a protected activity. Any adverse action (including intimidation, threats, or coercion) taken against an individual because the individual reported a concern constitutes retaliation and is strictly prohibited.
