To Make a Report

You may use either of the following two methods to submit a report:

Reporting Instructions:

  1. Gather supporting documentation.
    1. Meeting minutes
    2. Email copies
    3. Texts
    4. Other
  2. If you plan to make your report by phone, please ensure you are in a private place to discuss details. Note: you can make a report 24/7.
  3. After you complete your report you will be assigned a unique code called a "report key." Write down your report key and password and keep them in a safe place. After 5-6 business days, use your report key and password to check your report for feedback or questions.

Our Commitment

The mission of Children’s Hospital Colorado is to improve the health of children through the provision of high-quality, coordinated programs of patient care, education, research, and advocacy. As we strive to be the leader in providing the best healthcare outcomes for children, our expectations for the highest level of ethical conduct are ingrained in our culture and everything we do.

The Children’s Colorado Code of Conduct is simple: do the right thing. Our expectation is that all Team Members (employees, providers, contract personnel, vendors, trainees, students, volunteers, board members, and others) educate themselves and comply with all laws, regulations, and policies, and access resources as needed to resolve business and ethical dilemmas.

If you ever encounter a situation where the right choice is not clear, Children’s Colorado encourages you to do any of the following:
  • Refer to our policies and procedures, which can be found on My Children’s Colorado
  • Ask or speak to your team leader or leadership team
  • Contact Compliance and Business Ethics

We hold ourselves to the highest standards of character and integrity and encourage everyone in the organization to share concerns when they arise. It is everyone’s right and responsibility to report any activity that appears to violate the Children’s Colorado Code of Conduct or any laws, regulations, and/or Children’s Colorado policies.

In situations where you prefer to place a confidential report in confidence, you are encouraged to use this hotline, hosted by a third party hotline provider, EthicsPoint. You are encouraged to submit reports relating to violations of the Children’s Colorado Code of Conduct.

The information you provide will be sent to Children’s Colorado by EthicsPoint in a totally confidential manner with the option to maintain anonymity. Children’s Colorado assures you that your concerns will be heard and appropriately addressed.

What To Report

The Compliance and Ethics Reporting accommodates reporting of serious matters related, but not limited to, the following areas:

  • Illegal or Corrupt Behavior
  • Theft, fraud, bribery or other forms of corruption.
  • Intentional misappropriation or misuse of Children's Colorado resources, such as funds, supplies or other assets
  • Improper practices related to accounting, auditing, procurement, contracting, or tax, or customs violations
  • Business relationships with parties involved in illegal or sanctioned activity
  • Illegal employment practices including but not limited to:
    • Harassment
    • Discrimination
    • Retaliation

Compliance and Business Ethics promotes adherence to applicable federal, state, and local laws as well as Children's Colorado's policies and procedures. If you have concerns regarding a potential violation of any law, regulation, policy, and/or procedure, you are expected to take action. Reports can be made anonymously and securely. Children's Colorado has a Non-retaliation in Reporting Misconduct or Variances policy which makes clear there will be no retaliation or corrective action against any team member who reports a suspected violation in good faith.
