Our Code of Conduct provides a blueprint of ethics for all our interactions. It is part of the environment we are creating that encourages and fosters open and honest communication. We want everyone at PolarityTE to always have a way to speak up and to be heard. If there are times that violations of the Code of Conduct, policies, standards of practice or work instructions occur, these are the most important moments to speak up. In such situations, line managers, PolarityTE’s management, Compliance, Legal, or Human Resources can be engaged to address questions or issues. In situations where an anonymous, confidential option is needed or preferred, this website and PolarityTE’s helpline—hosted by EthicsPoint, a third-party provider—can be used to submit reports relating to violations (as stated in our Code of Conduct), as well as for guidance related to PolarityTE’s policies and procedures.

The information provided via this website or the helpline will be communicated by EthicsPoint to PolarityTE on a confidential and/or anonymous basis, if you so choose. You have our guarantee that your comments will be heard.

To Make a Report

You may use either of the following two methods to submit a report:

  • OR
  • Dial toll-free, within the United States, Guam, Puerto Rico and Canada:

After you complete your report you will be assigned a unique code called a "report key." Write down your report key and password and keep them in a safe place. After 5-6 business days, use your report key and password to check your report for feedback or questions.
