To Make a Report

You may use either of the following two methods to submit a report:

  • Select the "Make a Report" link at the top of this web page.
  • OR
  • Dial toll-free, within the United States: 1-833-875-7677

When you submit the report, you will be issued a Report Key. Please write it down and keep it in a safe place. We ask you to use this Report Key along with the password of your choosing to return to EthicsPoint through the website or telephone hotline as needed. You may return to EthicsPoint for the opportunity to review any Follow-up Questions or submit more information about this concern. In approximately 5-7 business days, when logged into the system, you will be able to view case updates and follow-up questions. If you provided your contact information, you will receive an email.

In situations where you prefer to place an anonymous report in confidence, you are encouraged to use this hotline, hosted by a third party hotline provider, EthicsPoint. You are encouraged to submit reports relating to violations stated in our Code of Conduct, as well as asking for guidance related to policies and procedure and providing positive suggestions and stories.

The information you provide will be sent to us by EthicsPoint on a totally confidential and anonymous basis if you should choose. You have our guarantee that your comments will be heard.

Privacy Breaches:

Consistent with HIPAA, we do not use, disclose or discuss patient-specific information, including patient financial information, with others unless it is necessary to serve the patient or required by law.

In accordance with our information privacy and security policies and procedures, which reflect HIPAA requirements, no OUH colleague, affiliated physician, or other health care partner has a right to any patient information other than that necessary to perform his or her job.
