After you complete your report you will be assigned a unique code called a "report key." Write down your report key and password and keep them in a safe place. After 5-6 business days, use your report key and password to check your report for feedback or questions.

All reports will be reviewed impartially, and relevant information will be reported in accordance with U.S. law. We ensure that your identity is treated confidentially.
You may report allegations of fraud/waste/abuse, misconduct, human trafficking, and child abuse/neglect/exploitation using this web portal, hosted by EthicsPoint. Creative abides by the “Do No Harm” principle as it relates to the people we serve. The information you provide will be sent to us by EthicsPoint on a confidential and anonymous basis if you should choose. You have our guarantee that your comments will be heard.

All reports should include detailed information about the alleged fraud or misconduct, including who committed it, what, where, when and how. Explain why you think the person or persons committed fraud or misconduct and who else might be aware of it. To assist in a thorough investigation, please refrain from reporting information that is overly broad or vague in nature.
