To Make a Report

The process for reporting a concern is simple:

First, tell us how we can contact you regarding this concern. If you do not want to provide your name, we can communicate with you through this website.
Next, tell us what happened. You will be asked a series of questions regarding your concern.
When you submit your report, you will be assigned a unique code called a "report key" and instructions for using this website to follow up on your report. Write down your report key and password and keep them in a safe place.
