To Make a Report

You may use either of the following two methods to submit a report:

  • Select the "Make a Report" link at the top of this web page.
  • OR
  • Dial toll-free, within the United States, Guam, Puerto Rico and Canada:

After you complete your report you will be assigned a unique code called a "report key." Write down your report key and password and keep them in a safe place. After 5-6 business days, use your report key and password to check your report for feedback or questions.

Our Commitment

Compliance is everyone’s responsibility. HealthCare Partners’ (HCP) commitment to ethical and legal conduct depends on our employees, providers, patients, and vendors who comprise the HCP community. Without assistance from our employees and others in the HCP community, it is impossible for us to be aware of every compliance issue that we need to address. Reporting non-compliance is essential to the success of our Compliance Program and in preventing, detecting , and correcting fraudulent activity and other misconduct.

Your Responsibility to Report

Our Code of Conduct contains general guidelines for conducting business with the highest standards of integrity and ethics. While our Code of Conduct cannot address every situation, HCP expects all of its employees to exercise good judgment and personal integrity.

We are devoted to promoting an environment where open and honest communication is the expectation, not the exception. We want each member of the HCP community to feel comfortable in reporting violations of policies or standards, laws, and regulations without fear of retaliation. HCP’s Whistleblower & Non-Retaliation Policy prohibits intimidation or retaliation against anyone who reports a compliance concern in good faith.

Anonymous Reporting Option

In situations where you prefer to place an anonymous report in confidence, you are encouraged to use HCP’s hotline, hosted by a third-party hotline provider, EthicsPoint. You are encouraged to submit reports relating to violations of our Code of Conduct, policies and procedures, federal and state laws and regulations, as well as asking for guidance.

The information you provide will be sent to us by EthicsPoint on a totally confidential and anonymous basis should you choose this option. HCP treats allegations of misconduct and other violations very seriously and will promptly respond to and thoroughly investigate all reports received through the hotline.
