After you complete your report, you will be assigned a unique code called a "report key." Write down your report key and password and keep them in a safe place. After 5-6 business days, use your report key and password to check your report for feedback or questions.

Members of the Brandeis community are encouraged to come forward with information regarding conduct they believe may violate a University policy. Such concerns can be submitted through this online portal by clicking “Make A Report.” For those who wish to speak directly with a member of the Brandeis community, such conduct can be reported by anyone directly to the Office of Equal Opportunity (OEO) (781-736-4806 or oeo@brandeis.edu). Students can also report to the Dean of Students Office (781-736-3600 or deanofstudentsoffice@brandeis.edu) and Employees (staff or faculty) can report to Human Resources (781-736-4474 or humanresources@brandeis.edu).
To submit a report, please click on “Make A Report” at the top of the page to access a list of reporting categories. After selecting a category, you will be taken to a reporting form. Before submitting your report, you will be asked to create a password. After submitting the report, you will be given a report key. It is important that you write down the password and report key to be able to access your report again. This is especially important if you choose to report anonymously, as this reporting portal will allow us to communicate with you while still maintaining your anonymity. You should use the “Follow up on A Report” link at the top of the page within five (5) business days after submitting your report using your password and report key to see if there is any communication from Brandeis regarding the information submitted. Please note that, without being able to follow up on a report, Brandeis typically is unable to meaningfully respond to concerns that are reported anonymously.
Please know that information submitted through this online reporting portal will be handled with the utmost privacy and discretion, and will only be shared with those administrators within the institution who can address these concerns. Typically that would include either the Office of Equal Opportunity or Human Resources depending on who is involved in the conduct.
