Pacific Clinics offers mental health services, substance use disorder treatment, and other services to children, youth, transitional age youth, adults, older adults, and to families who are Medi-Cal eligible. We pride ourselves in developing and delivering community-based, culturally sensitive programs.

Pacific Clinics is committed to an environment where open, honest communications are the expectation, not the exception. We want you to feel comfortable in us in instances where you believe there has been a violation of the law, rule, regulation, policy or Standards of Conduct, or you believe there has been misconduct, have a concern or complaint.

You may submit reports relating to actual or perceived violations or of misconduct, or to provide suggestions and positive stories, etc. For Pacific Clinics Staff, you encouraged to ask for guidance related to the Agency’s policies and procedure or practices through the Hotline. Simply Make a Report on this web page or call the Hotline at (844) 711-0266. The Hotline is available 24-hours a day, 7-days a week, 365-days a year. You can also click the Submit a Complaint link on Pacific Clinics web page (Staff may also access the Hotline on PC Web by clicking on the Submit a Complaint link). In situations where you prefer to place an anonymous, confidential report, you are encouraged to use this Hotline.

The Hotline is hosted by EthicsPoint. The information you provide will be sent to Pacific Clinics by EthicsPoint through a secure server on a totally confidential and anonymous basis if you so choose. You have our guarantee that you will be heard.

To Make a Report

After you complete your report you will be assigned a unique code called a "report key." Write down your report key and password and keep them in a safe place. After 5-6 business days, use your report key and password to check your report for feedback or questions.
