Equitable Ethics and Misconduct Reporting Program

Equitable Financial Life Insurance Company’s Ethics and Misconduct Reporting Program provides two confidential channels through which you can report concerns involving, or alleged violations of, the Code of Business Conduct and Ethics Policy, Reporting Misconduct Policy, or those related to accounting, auditing and internal financial controls.

To submit a report, please visit either the Report Online or Report by Phone tab, above. Both the web-based form and the hotline allow you to submit your report anonymously, should you choose to do so. After you complete your report you will be assigned a unique code called a “report key.” Write down your report key and password and keep them in a safe place. After 5-6 business days, you may use your report key and password to check your report for follow up questions.

Please note: to make a report concerning an alleged fraud, you may do so anonymously by calling the Fraud Hotline at (212) 314-6600 or by completing and submitting the online form available at https://equitable.com/customer-service/consumer-safety-awareness/report-fraud. You may also report your concern by e-mailing ReportFraud@equitable.com.
