Our Commitment

Perceptive Informatics, LLC and its affiliates d/b/a Calyx (“Calyx”) is an organization with strong values of responsibility and integrity. Our Code of Business Conduct and Ethics (“Code”) contains general guidelines for conducting business with the highest standards of ethics. Our Code, together with all other company policies, outline expectations for all Calyx employees. Included in the Code is the responsibility of all employees and suppliers to report potential violations of company policies and standards about which they may become aware.

Calyx is committed to an environment where open, honest communications are the expectation, not the exception. We want you to feel comfortable in approaching your supervisor, line management, Legal, the Chief Compliance Officer, or Human Resources in instances where you believe violations of company policies or standards may have occurred, or if you become aware of an ethical issue or question.

If you feel that you are unable to report a concern to any of the above individuals, you are encouraged to use this hotline, hosted by a third-party hotline provider, EthicsPoint. The information you provide will be sent confidentially to the Calyx Legal Department by EthicsPoint. See the Speak-Up FAQs for more information.

To Make a Report

After you complete your report you will be assigned a unique code called a "report key." Write down your report key and password and keep them in a safe place. After 5-6 business days, use your report key and password to check your report for feedback or questions.
