If you are not comfortable reporting concerns through normal management channels, you may submit a report confidentially and anonymously using this hotline, which is hosted by EthicsPoint, an independent third party provider. Reports may include:

For incidents requiring immediate attention—such as an imminent threat or an act of violence—please call 911 immediately.
Urgent, non-emergency incidentsIf you are reporting an incident that is not an imminent threat but still requires prompt attention, please call the Harris Health Hotline at 844-565-0621. Inform the representative that this is a high priority call. The representative will notify the appropriate personnel.
Non-urgent incidentsAll other non-urgent incidents may be reported through this website.

Once your report is complete, you will be assigned a unique report key and password. Please write them down and keep them in a safe place. After 5-6 business days, you may use your report key and password to check your report for feedback or follow up questions.
