Participate in Our Speak Up Program: Speak Up is a way for everyone to participate in the Cerapedics Compliance program! Speak Up encourages you to ask questions, raise concerns, report potential misconduct, and disclose potential conflicts of interest. One way to Speak Up, is to use our hotline, hosted by a third-party hotline provider, EthicsPoint. You are encouraged to submit reports relating to violations stated in our Code of Business Conduct and Ethics, as well as asking for guidance related to policies and procedures and providing positive suggestions.
For Employees: Every employee should feel comfortable Speaking Up about Ethics and Integrity, including submitting reports of concerns and/or potential violations of Company policies without fear of retaliation. There are many channels you can use to Speak Up. You can reach out directly to Compliance, your manager, HR, or leadership. In situations where you prefer to place a formal report directly to Compliance (and/or remain anonymous), you are encouraged to use this hotline, hosted by a third-party hotline provider, EthicsPoint.
For Others: Every employee, agent, consultant, customer, and patient can Speak Up for the values of Cerapedics! Our hotline is designed to receive complaints of wrongdoing from third parties, including anonymously. It is up to all of us to live the Cerapedics values and protect the integrity of everything we do. When we work together daily to maintain a safe, ethical, and compliant work environment, we ensure we always Lead with Integrity!

To Make a Report
You may use either of the following two methods to submit a report:

By Phone:
After you complete your report you will be assigned a unique code called a "report key." Write down your report key and password and keep them in a safe place. After 5-6 business days, use your report key and password to check your report for feedback or questions.
