If you are reporting a concern regarding the misuse of public assistance, please call 1-888-372-8301 or access the fraud reporting website at State of Oregon: Abuse - Report Fraud. You may also email the Department of Human Services directly at DHS.FRAUD-INVESTIGATIONS@dhsoha.state.or.us.
Public assistance would include any of the following Oregon sponsored social welfare programs: Food stamps; Temporary Assistance for Needy Families (TANF); Oregon Trail card, which is used for Electronic Benefits Transfer (EBT); Childcare benefits; housing assistance; or DHS Contractors.
In situations where you prefer to place an anonymous report in confidence, the information you provide will be sent to us by EthicsPoint on a totally confidential and anonymous basis. You have our guarantee that your comments will be heard.

After you complete your report you will be assigned a unique code called a "report key." Write down your report key and password and keep them in a safe place. After 5-6 business days, use your report key and password to check your report for feedback or questions.
