If you filed before 11/30/21 please select “Report filed before 11/30/21” (date of the upgrade). On the page that appears you will create a Follow-Up Report by entering the Original Report number and any additional information you would like to submit in the text box. The Report number was on the confirmation email you received when you originally filed. You will also need to create a password prior to submitting the report. Please remember to keep this password along with the report number to allow for future communication.
If you filed after 11/30/21, including if you have already created a Follow-Up Report, please select "Report filed after 11/30/21" and enter your Report Key and Password to check on the status of your concern.
