By giving us information, you acknowledge that it may be shared with the relevant regulatory agencies. Additionally, your information may be shared with Dogwood Health Trust and—unless you request otherwise—with HCA’s Mission Health System. You may also choose to remain anonymous; however, that choice could hamper a full review of the information you provide.
