Follow-up on a Report Our Values and Ethics at Work

To Make a Report

You may use either of the following two methods to submit a report:

  • Call 1-800-261-5607 (toll free), or
  • To make an online report choose the state where you are currently employed from the options below.

After you complete your report you will be assigned a unique code called a "report key." Write down your report key and password and keep them in a safe place. After 5-6 business days, use your report key and password to check your report for feedback or questions.

EthicsPoint is NOT a 911 or Emergency Service:
Do not use this site to report events presenting an immediate threat to life or property. Reports submitted through this service may not receive an immediate response. If you require emergency assistance, please contact your local authorities.

To support our core values and standards of conduct, CHI provides you with the Ethics at Work Line. The Ethics at Work Line allows you to ask a question or report an actual or suspected violation of law, regulation, policy or procedure. Calls or reports to the Ethics at Work Line are received by trained staff who forward your questions or concerns to your local corporate responsibility officer or other appropriate independent party. When calling or electronically reporting a concern, you may choose to remain anonymous. If you choose to identify yourself, there is no guarantee your identity will remain confidential. However, if you do choose to identify yourself, it may be easier to follow up directly with you to provide a direct response.
