If you wish to remain anonymous, please do not report information in a manner that may personally identify you. For example:

• Do not include your relationship to persons identified in your report
• Do not include your physical location relative to persons or incidents in your report

Service Provider does not warrant the accuracy or reliability of the information collected, nor does it investigate or resolve issues or concerns submitted. You, the reporter, understand and acknowledge that the Service Provider’s sole obligation to you is to receive your reported information and deliver the reported information to our Client. Please be advised, that the Client is responsible to evaluate the information submitted and to comply with all applicable local, state or federal laws relating to the investigation and protection of the information submitted. Any false or misleading information submitted may lead to civil and/or criminal prosecution. Service Provider will incur no liability as a result of the action or inaction of the organization about which you are filing a Report.

Service Provider is Safe Harbour Certified with the United States Department of Commerce. By placing your report with Service Provider you acknowledge and agree that our Client may process, store and otherwise use the reported information as they deem appropriate to investigate and take appropriate action in response to your report.
