The Reporting Process
Step 1: Complete a Report
You may use either of the following two methods to submit a report:
-
By Telephone:
Dial (toll free in the U.S & Canada) - or
-
Online:
Select the "Make a Report" link at the top of this web page.
Follow the on-screen or compliance specialist instructions to complete a report.
Please be as thorough as you can. Remember you may either choose to give your name and contact information, or to remain anonymous.
- Specify what kind of report you’d like to make from the available choices. If the choices do not match your situation exactly, pick one that is closest to the situation you wish to report.
- Please read or listen to the anonymity information very carefully. It helps to ensure that you do not compromise your identity during the reporting process. You DO NOT have to complete the location information. If you wish to remain anonymous, simply skip this section.
-
You will now complete the report, providing information about the incident, including:
- Who: persons engaged in the incident, including titles if known;
- What: what specifically occurred;
- Where: the location of the incident: and
- When: the time and duration of the incident
- Next, you will be asked to create a password. Then the EthicsPoint system will generate an identification code called a “Report Key”. Write them both down and keep them in a safe place. You will need them to follow-up later, to see if Peckham needs clarification, or if you ever want to review or amend your claim.
Step 2: Investigation Begins
The reports you file are available only to specific individuals within the company who are charged with evaluating the type of violation and location of incident. These individuals at Peckham Inc. include the Compliance Officer, Human Resource Department, the C.E.O., and may include a Board member when necessary. Each of these recipients has had training and is committed to keeping these reports in utmost confidence.
An employee can be assured any incident report will be handled as confidentially as possible, respectfully, and without fear of retaliation, reprisal, or embarrassment.
Step 3: Follow-Up
Three to five (3-5) business days after you complete your report, please return to the EthicsPoint system. Click on "Follow-up on a Report" to log in using your "Password" and "Report Key" to see if Peckham has any follow-up questions or requests. You may need to return to the system more than once to answer questions or to provide more information.
