To Make a Report

You may use either of the following two methods to submit a report:

  • Dial toll-free, within the United States, Guam, Puerto Rico and Canada:

After you complete your report you will be assigned a unique code called a "report key." Write down your report key and password and keep them in a safe place. After 5-7 business days, use your report key and password to check your report for feedback or questions.

EthicsPoint is NOT a 911 or Emergency Service:
Do not use this site to report events presenting an immediate threat to life or property. Reports submitted through this service may not receive an immediate response. If you require emergency assistance, please contact your local authorities.

To enable the fulfillment of its mission, the Diocese of San Jose is committed to transparent dealings with the people it serves and with the members of the wider community. Accountability is a very important element of this transparency. We attempt to involve a large and diversified number of people in consultative bodies at every level of the Church structure, through finance and pastoral councils and many other consultative groups.

Our goal is to protect the people who interface with the Diocese’s parishes, schools and various ministries and agencies, as well as being faithful stewards of the assets entrusted to our care, whether they be financial, physical or informational.

We have selected EthicsPoint to provide us with an anonymous and confidential method to hear your suggestions, concerns or the report of misconduct.

The information you provide will be sent to us by EthicsPoint on a totally confidential and anonymous basis.

EthicsPoint will accept reports of misconduct through its website at the links on this page or the hotline telephone number 1-888-325-7863 and provide a complete report to the Diocese of San Jose. We will then continue the investigation and determine how best to resolve the issues. You will receive feedback on the progress of the investigation.

To report Safe Environment policy violations or allegations of sexual misconduct, you may contact the Director of the Office for the Protection of Children and Vulnerable Adults, Anthony Gonzalez, directly at 408-983-0113.
